Clinic SEO Case Study: How We Took a Kuwait Clinic to #1 on Google and Into AI Answers in 2 Months

Dr. Fahad Alebrahim’s clinic now ranks #1 on Google in Kuwait for 9 of the doctor searches that matter most to it. In August 2026, the month before VOCTOS started, it ranked #1 for none. One month into the work, searches such as افضل دكتور جهاز هضمي في الكويت (best gastroenterologist in Kuwait), افضل دكتور معدة (best stomach doctor) and طبيب جهاز هضمي (gastroenterologist) had all reached the top spot.
AI search moved with Google. Asked “who is the best gastroenterologist in Kuwait?” on 24 October 2026, Gemini answered with Dr. Fahad Alebrahim’s clinic, and Google’s AI Overview for the same Arabic search named him among the country’s leading consultants. Below are the 2 months of work behind those results, with the data and screenshots for each number.
- VOCTOS began SEO and GEO for drfahadclinic.com in September 2026, tracking 48 Arabic searches on Google in Kuwait.
- Searches ranking #1 went from 0 in August to 9 in September 2026, and searches in the top 3 from 2 to 11.
- From 1 September to 22 October 2026, Google clicks rose 51% to 6.27K and impressions rose 20.7% to 818K against the previous 52 days.
- Daily impressions in Google’s AI features tripled, from 1,987 on 3 September to 5,998 on 14 October 2026.
- Gemini recommends the clinic for “best gastroenterologist in Kuwait”, and Google’s AI Overview names Dr. Fahad Alebrahim among the leading consultants.
Where did Dr. Fahad’s clinic start in August 2026?
The clinic started with strong medical credentials and weak search visibility. In August 2026, none of 48 tracked Arabic searches in Kuwait ranked #1 on Google, only 2 sat in the top 3, and core searches such as طبيب جهاز هضمي (gastroenterologist) ranked 22nd. A keyword gap analysis showed direct competitors ahead on the high-volume terms.
That gap had nothing to do with the doctor. Dr. Fahad Alebrahim is a consultant in gastroenterology, hepatology, endoscopy and obesity treatment, holds American board certification in internal medicine and a gastroenterology fellowship from McMaster University in Canada, and heads Kuwait’s national programme for early detection of colon cancer. His clinic is at Al Hashel Medical Complex in Salmiya. The website simply was not turning that authority into rankings.
The first audit showed why. The XML sitemap returned a 404 error, so Google had no clean list of the site’s pages. The homepage carried structured data errors. More than 300 articles had no category, a duplicate H1 ran across the whole site, and one article named Al Jahra as a service area the clinic does not cover. A full link audit later counted 594 link issues, from hreflang conflicts to internal links passing through 301 redirects.
What did VOCTOS do in the first 2 months?
We split the 2 months into two sprints. The first fixed what blocked Google and set the keyword targets; the second published content, cleaned the link graph and prepared the site for AI assistants. The September items are documented in the work report delivered to the clinic.
| Sprint | Work | Result |
|---|---|---|
| September, sprint 1 | Arabic keyword research, keyword mapping, keyword gap analysis | Each topic cluster tied to one target URL |
| September, sprint 1 | Meta titles and descriptions for the main pages | Every main page targets one primary search |
| September, sprint 1 | Sitemap and robots.txt check | Sitemap fixed from 404 to 200 |
| September, sprint 1 | Schema review | Errors fixed, MedicalClinic schema added to the homepage |
| September, sprint 1 | Content plan for 8 articles, Instagram audit | 8 briefs built around search intent |
| September, sprint 2 | 8 new articles published, content audit of older articles | Internal links, keyword fixes, a duplicate article redirected |
| September, sprint 2 | Comprehensive link audit | 594 link issues fixed |
| September, sprint 2 | Duplicate H1 fix, categories for 300+ articles, blog page redesign | Cleaner structure for users and crawlers |
| September, sprint 2 | llms.txt file, GA4 key events | AI-readable clinic summary, phone and WhatsApp contacts tracked |
| October | Backlink building, keyword cannibalisation audit, AI Overview tracking | Authority and overlap fixes under way |
Two of these deserve a note. The sitemap fix mattered more than its size suggests: a 404 sitemap means Google relies only on links to find pages, which slows the indexing of every new article. And the llms.txt file gives ChatGPT, Gemini and other assistants a plain summary of who the doctor is and which articles exist, so they have a clean source to quote.
How did VOCTOS fix overlapping content, schema and the two-domain problem?
Beyond the technical errors, three problems were holding the clinic back: content that competed with itself, schema that search engines could not trust, and two websites for the same clinic. Each one split authority that should have gone to a single page.
- Thin and duplicate content. Many of the clinic’s 305 articles were thin or covered the same topic more than once. We ran a keyword cannibalisation audit across the blog and the 26 service pages, then cleaned up the low-quality and near-duplicate articles so each topic has one page that owns it.
- Articles competing with service pages. Some blog posts ranked for the same searches as the clinic’s own service pages. We reassigned those searches so the service page owns the booking intent and the article supports it with internal links.
- Schema errors. We fixed the existing errors and added Physician, MedicalClinic, LocalBusiness and FAQPage schema, so Google and AI assistants read the same facts about the doctor and the clinic.
- Two websites for one clinic. The practice ran drfahadclinic.com and drfahadalebrahimclinic.com side by side, which split Google’s signals and confused which site should rank. We brought both domains into Search Console and started consolidating them into one primary site.
- Conversions, not just traffic. We improved the services page experience and added inbound calls to action to articles and pages, sending readers to WhatsApp and phone. Both contacts are now tracked as GA4 key events, so the clinic sees bookings, not only visits.
The baseline for all of this was a full Screaming Frog crawl of the site’s 333 URLs, an audit of all 26 service pages, and a backlink gap analysis against three competitors that led to 30 new backlinks plus 10 reverse-engineered links in the first month.
Which searches reached #1 on Google in Kuwait?
Nine tracked searches reached #1 in Kuwait in the September 2026 rank check, against none in August. Searches in the top 3 went from 2 to 11, and searches on page 1 from 11 to 16. The biggest jumps came on commercial doctor searches, the ones a patient types when choosing who to book.
| Search (Kuwait) | Intent | Aug 2026 | Sep 2026 |
|---|---|---|---|
| افضل دكتور قولون في الكويت (best colon doctor in Kuwait) | Informational | 3 | 1 |
| دكتور لعلاج السمنة (obesity treatment doctor) | Commercial | Not in top 50 | 3 |

A caution on reading this table: one month is a short window, and positions for local doctor searches move. We will judge the rankings again at the 3 and 6 month marks before calling any of them settled.
How much did Google traffic grow in 2 months?
From 1 September to 22 October 2026, drfahadclinic.com earned 6.27K clicks and 818K impressions in Google Search. Against the 52 days before, 11 July to 31 August, that is +51% clicks and +20.7% impressions, with CTR up from 0.6% to 0.8% and average position up from 8 to 7. Daily clicks more than doubled, from about 75 to about 170.
| Search Console metric | 11 Jul to 31 Aug 2026 | 1 Sep to 22 Oct 2026 | Change |
|---|---|---|---|
| Clicks | 4.15K | 6.27K | +51.0% |

Does Dr. Fahad’s clinic show up in AI answers in Kuwait?
Yes, in both Gemini and Google’s AI Overview. On 24 October 2026, Gemini answered “who is the best gastroenterologist in Kuwait?” with Dr. Fahad Alebrahim’s clinic as its recommendation. Google’s AI Overview for the same Arabic search listed him among the leading consultants: first in one check and second in another the same day.

The AI Overview repeats a detail from the clinic’s own site: Dr. Fahad Alebrahim heads Kuwait’s national colon cancer early detection programme. That is the kind of specific, verifiable fact AI systems reuse, so it pays to state it the same way everywhere the clinic appears.
Semrush’s worldwide view adds the cross-engine picture. In October 2026 it counts 53 drfahadclinic.com pages cited by AI assistants: 29 in Google AI Overviews, 23 in Gemini, 20 in AI Mode and 1 in ChatGPT, with an AI Visibility score of 14. Semrush puts the clinic’s traffic share in Kuwait at 18%, and Semrush’s worldwide organic traffic estimate rose from 2,025 visits on 28 August to 2,408 on 19 October 2026.

Search Console shows the same trend at scale. Daily impressions in Google’s AI features tripled, from 1,987 on 3 September to 5,998 on 14 October 2026, and reached 326K over the 3 months to 22 October.

One caveat: most of those AI impressions come from outside Kuwait, because the clinic’s health articles are read across the Arab world. Kuwait ranked seventh by AI impressions in the period, with 13,761. The local visibility that brings patients through the door is the Gemini and AI Overview result above, and that is what we track every month.
Our read: why a specialist clinic can win AI answers quickly
Our view is that a specialist with real credentials can move fast in AI search, because the hard part, authority, already exists. The work is to make it legible: one clear page per service, consistent facts about the doctor across the site, schema that states them, and nothing broken that stops crawlers from reading it.
AI assistants do not invent a doctor’s reputation. They repeat the facts a site states clearly and consistently. Fix the site, and the reputation carries.
What comes next?
The doctor searches are won; the health topics are not yet. In September 2026, informational searches such as ارتجاع المريء (acid reflux), القولون العصبي (IBS), الكبد الدهني (fatty liver) and تنظير القولون (colonoscopy) were still outside the top 50. The next stage builds topic clusters for each, adds backlinks from Kuwaiti and Gulf health sources, and resolves the keyword cannibalisation found in the October audit.
Clinic SEO questions we hear most
My clinic has a great doctor but ranks on page 2 for “best doctor” searches.
That was Dr. Fahad’s position in August 2026, with “طبيب جهاز هضمي” (gastroenterologist) at 22. The usual causes are technical: a broken sitemap, schema errors and no single page per search intent. Fixing those and mapping one search to one page took that search to #1 within a month.
How long does SEO take for a clinic in Kuwait?
For commercial doctor searches, the first #1 rankings for this clinic came within one month of starting in September 2026, and Google clicks were up 51% after 52 days. Informational health topics such as acid reflux or fatty liver take longer, typically several months of content and links.
ChatGPT and Gemini recommend other doctors, not me.
AI assistants repeat facts they can verify across your site and other sources. Make the doctor’s credentials, specialties and clinic details consistent on every page, add MedicalClinic schema, and publish clear answers to patient questions. Within 2 months, Gemini was naming Dr. Fahad’s clinic for best gastroenterologist in Kuwait.
Can I measure how many patients SEO brings in?
Yes, if contacts are tracked. For this clinic we set up GA4 key events for phone calls and WhatsApp messages, so every booking enquiry from search can be counted rather than estimated.
So what does a clinic need to win “best doctor” searches?
The useful question is not whether a clinic can rank for “best doctor”. It is whether Google and AI assistants can read, trust and repeat what makes that doctor the right choice. For Dr. Fahad Alebrahim, making that legible took 2 months and produced 9 #1 rankings in Kuwait.
If your clinic already ranks for its own name but not for the specialty in your city, start with the technical base and the keyword map. If it already ranks well, the next lever is AI answers.
