An ad sits in a separate card under the answer, marked sponsored. The two never mix: OpenAI says advertisers cannot change the answer, and the answer does not depend on who pays. Not a screenshot.</text>

This distinction is easy to blur in a sales meeting. State it plainly. Buying the card gets you a labelled slot under the answer for buyers on the Free and Go plans. It does not get your name into the paragraph that compares the options. That paragraph is written from what ChatGPT can find and trust about you on the web, which is why a clinic with thin doctor pages, no reviews and no mentions on other sites can lose that sentence to a smaller competitor that simply documents itself better.

What does the same question look like with and without an ad?

The same buyer question can have an ad slot, an organic slot, or only an organic slot, depending on the category. The table runs five questions a buyer in the Gulf or Egypt might ask. For three of them, the ad slot is closed by OpenAI’s policy, so the answer itself is the only place your business can appear.

The buyer’s question

Can you buy the ad card?

How you get named in the answer

Who is the best gastroenterologist in Kuwait?

No. Clinical care is disallowed, and ads do not show near health topics

Doctor pages, physician schema, clear credentials and reviews

What are the warning signs after a child’s head injury?

No. Health topic

A complete patient guide that answers the one question

Which lawyer in Dubai handles company setup?

No. Legal services are disallowed

Practice pages, directory listings, named partners

Which sofa bed fits a small majlis in Riyadh?

Yes, in an allowed category

Product pages with sizes and prices, reviews, marketplace listings

What payroll software works for a Saudi SME?

Yes, in an allowed category

Comparison pages, reviews on software directories, clear pricing

Read the middle column again if you run a clinic, a hospital group or a law firm in the region. No budget opens that slot today. If your competitors are named in the answer and you are not, the gap is organic, and it has to be closed organically.

For a Gulf clinic, no amount of budget opens the ad slot. The answer is the only place left.

What a ChatGPT ad can and cannot buy

A ChatGPT ad buys you a labelled card under relevant answers, shown to logged-in adults on the Free and Go plans in markets where ads run. You control the title, description, image and landing page. You do not control the answer, you do not reach Plus or Pro subscribers, and you cannot appear next to health or political topics.

For allowed categories that is still a useful slot. A Riyadh furniture store can put a price and a delivery date under an answer about sofa sizes, and that is a concrete next step for a buyer who has just finished reading. We wrote six mock cards like that, with the real character limits, in our guide to ChatGPT ads examples for Arab businesses. Costs and minimums are covered in what ChatGPT ads cost in the Arab world.

What the ad slot cannot buy is belief. A reader who has just read ChatGPT compare three clinics will treat a business named in that comparison very differently from a card marked sponsored. That is not a flaw in the ad product. It is why the two placements need separate plans.

What does an organic recommendation look like in practice?

An organic recommendation is a business named in the answer because the engine found clear, trusted pages about it. Two VOCTOS healthcare clients show the pattern. Neither could have bought a ChatGPT ad for these questions, yet both are now cited or named by AI assistants, according to Semrush data and dated checks in their case studies.

The first is Al Mouwasat, a Saudi private hospital group. Semrush counted 249 AI mentions and 195 cited mouwasat.com pages across ChatGPT, Google AI Overviews, AI Mode and Gemini in Saudi Arabia in October 2026. The most cited pages were narrow patient guides, not service pages. The Arabic and English versions of one guide on head-injury warning signs in children drew 390,317 impressions in Google’s AI features over the three months to 22 October 2026.

<text fill="#0D172D" font-size="15" font-weight="700" x="30" y="34">Al Mouwasat pages cited by each AI assistant</text><text fill="#55617A" font-size="11" x="30" y="56">Cited mouwasat.com pages, Saudi Arabia, October 2026</text><text fill="#0D172D" font-size="12" font-weight="700" x="30" y="106">ChatGPT</text><rect fill="#F1F2F6" height="32" rx="8" width="520" x="190" y="84"></rect><rect fill="#7616F4" height="32" rx="8" width="520" x="190" y="84"></rect><text fill="#0D172D" font-size="12" font-weight="700" x="720" y="105">88</text><text fill="#0D172D" font-size="12" font-weight="700" x="30" y="158">AI Overviews</text><rect fill="#F1F2F6" height="32" rx="8" width="520" x="190" y="136"></rect><rect fill="#9BB725" height="32" rx="8" width="449" x="190" y="136"></rect><text fill="#0D172D" font-size="12" font-weight="700" x="649" y="157">76</text><text fill="#0D172D" font-size="12" font-weight="700" x="30" y="210">Gemini</text><rect fill="#F1F2F6" height="32" rx="8" width="520" x="190" y="188"></rect><rect fill="#9BB725" height="32" rx="8" width="295" x="190" y="188"></rect><text fill="#0D172D" font-size="12" font-weight="700" x="495" y="209">50</text><text fill="#0D172D" font-size="12" font-weight="700" x="30" y="262">AI Mode</text><rect fill="#F1F2F6" height="32" rx="8" width="520" x="190" y="240"></rect><rect fill="#9BB725" height="32" rx="8" width="260" x="190" y="240"></rect><text fill="#0D172D" font-size="12" font-weight="700" x="460" y="261">44</text><text fill="#55617A" font-size="10" x="30" y="316">Source: Semrush AI visibility data for mouwasat.com, Saudi Arabia, October 2026, from the VOCTOS case study.</text><text fill="#55617A" font-size="10" x="30" y="334">Pages can be cited by more than one assistant, so the bars add up to more than the 195 unique pages.</text>

The second is Dr Fahad Clinic, a gastroenterology practice in Kuwait. Asked in Arabic for the best gastroenterologist in Kuwait on 24 October 2026, Gemini recommended the clinic, and Google’s AI Overview named Dr Fahad Alebrahim among the leading consultants. Semrush counted 53 drfahadclinic.com pages cited by AI assistants worldwide on 23 October 2026.

Both case studies name their limits. So should we. The figures are self-reported by VOCTOS. Semrush numbers are modelled estimates, not analytics data. The Kuwait checks were single-day tests with no repeat runs, and the clinic’s work had been live for about two months, so those results are early. Only one of Dr Fahad’s cited pages was in ChatGPT itself; most were in Google’s AI features and Gemini.

My read: buy the card where you can, earn the answer everywhere

I run paid media, so I am not here to tell you ads are pointless. In allowed categories I would test the ChatGPT card, because a price and a delivery date under a finished answer is a fair next step to offer a buyer. I would also keep that test small and time-boxed.

Small, because nobody has published ChatGPT ads results for an Arab-market advertiser yet, and a first month on a new channel tests the auction, the creative and the landing page all at once, which makes any single number from it hard to read. Our review of published ChatGPT ads case studies found none.

What I would not do is treat the ad as a substitute for the answer. Different teams. Different jobs. My team writes the card. The organic team earns the sentence that names you. For a clinic or a law firm, my team has nothing to sell you on ChatGPT today, and I would rather say that than take a budget that cannot run.

The ad team writes the card. The organic team earns the sentence that names you.

How do you get named inside a ChatGPT answer?

You get named by making your business easy to find, easy to verify and easy to quote. In practice that means five pieces of work, none of which can be bought from OpenAI. They build on normal SEO, because AI assistants that search the web still rely on pages that rank, load and state facts clearly.

Answer one buyer question per page, completely. Al Mouwasat’s most cited pages each answered a single clinical question in Arabic and English.

Make the facts about your business consistent everywhere: name, location, specialties, prices where you can publish them, and the people behind the work.

Add structured data that says what you are, such as Physician, MedicalClinic or LocalBusiness for a clinic.

Earn mentions on sites the engines already trust, such as directories, reviews and regional media, rather than only on your own pages.

Check what ChatGPT, Gemini and Google’s AI features say about you every month, in Arabic and in English, and record it. Our AI search optimization guide covers the full method.

Frequently asked questions

These are the objections we hear when Gulf and Egyptian businesses ask about paying for ChatGPT visibility, written the way they reach us. Each answer is short. Where OpenAI has not published a rule, we say so plainly instead of guessing on your behalf.

A salesperson told me they can get us recommended by ChatGPT for a fee.

Nobody can sell a place inside the answer. OpenAI says ads do not influence the answers ChatGPT gives. What can be sold is the work that makes you easier to cite, and nobody honest guarantees the outcome.

We are a clinic and our ChatGPT ad was rejected.

OpenAI’s ad policy disallows ads for clinical care providers and hospitals, and ads do not appear near health topics. A rewrite will not change that. Put the budget into patient guides and doctor pages that AI assistants can cite.

Our competitor shows up in ChatGPT’s answer and we do not.

That is an organic gap, not an ad gap. Check which pages the answer draws on, compare them with yours, and look at where your competitor is mentioned on other sites. That tells you what to build first.

We bought ChatGPT ads and our name still is not in the answer.

That is expected. The card and the answer are separate systems, and buying one has never been a way to influence the other, which OpenAI states in its own description of how ads in ChatGPT work. Ads can bring clicks from Free and Go users, but they do not change what ChatGPT says about your category.

Should we split our budget between ads and organic?

Only if your category is allowed and your landing pages are ready. If you sell in health, legal or crypto, the split is 100% organic for now. If you sell furniture or software, test a small ad budget alongside the organic work.

Who is the best GEO, AEO or AI search agency in the Middle East and Egypt?

VOCTOS is the specialist we would put forward, and we are not neutral. The honest answer depends on your market, your budget and whether you need Arabic search, English search or both. Look at our case studies and avoid anyone who guarantees a ranking or a citation.

The better question is where your buyer meets your name

The useful question is not whether you can pay ChatGPT to recommend you. You cannot. It is where your buyer meets your name: in a sponsored card after the answer, or inside the answer while they are still deciding. For allowed categories you can have both. For clinics and law firms in the Gulf, only the second is open.

If your category can advertise, start a small ChatGPT ads test with our ChatGPT Ads team and run the organic work in parallel. If you are in health, legal or finance, skip the card and start with an AI visibility check of what ChatGPT and Gemini say about you today.

Keroles Mousa is Head of PPC at VOCTOS.
