That was Semrush&#8217;s estimate at the December 2025 peak, up from 182,399 in November 2023, the month before VOCTOS took over the hospital&#8217;s SEO. The traffic that matters most to a hospital grew faster still: non-brand visits, from people who had not yet decided where to go, rose 2.9X.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rankings moved with it. Of the 107 high-intent Arabic keywords we track every month on Google.com.sa, 4 sat on Google&#8217;s first page in December 2023. By October 2025, 72 did, and 12 of them ranked #1. Google&#8217;s AI features now show the hospital&#8217;s patient guides 1.72 million times a quarter. In November 2023, Semrush estimated 182,399 monthly organic visits from Saudi Arabia, and 171,228 of them, about 94%, came from people searching for the hospital by name. On 107 high-intent Arabic keywords, only 4 ranked on Google&#8217;s first page and none in the top 3.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Al Mouwasat is a Saudi private hospital group with hospitals in cities including Dammam, Madinah and Jubail, and a bilingual website at mouwasat.com. A group that size never lacks people typing its name into Google. Twelve reached #1. The two versions of the head injury guide alone drew 390,317 AI impressions, about 23% of the 1.72M total, and five vitamin D pages together drew 250,378.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Three things stand out. The winning pages are narrow: one question, one page, a direct answer near the top. They exist in Arabic and English, so a bilingual market gives each topic two chances to be cited. And they come in clusters, where several related pages on the same subject reinforce each other instead of competing.<\/p>\n\n\n\n<blockquote style=\"margin:32px 0;padding:22px 26px;border-left:6px solid #BAE034;background:#0D172D;color:#fff;border-radius:0 16px 16px 0;font-family:Unbounded, system-ui, sans-serif;font-size:18px;line-height:1.55;\">A hospital&#8217;s service pages win the click. Its patient guides win the citation. A search strategy for healthcare now needs both, built on purpose.<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>For a hospital, the practical rule is simple: write one complete answer per clinical question, publish it in Arabic and English, and link the related answers into a cluster.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What did not work?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Not every keyword held. Internal medicine searches that sat at #1 for most of 2024 slid in 2025: <bdi lang=\"ar\">\u0627\u0644\u0627\u0645\u0631\u0627\u0636 \u0627\u0644\u0628\u0627\u0637\u0646\u064a\u0629<\/bdi> (internal medicine) went from #1 to 23, <bdi lang=\"ar\">\u0623\u0634\u0647\u0631 \u0623\u0645\u0631\u0627\u0636 \u0627\u0644\u0628\u0627\u0637\u0646\u0629<\/bdi> from #1 to 30, and <bdi lang=\"ar\">\u0623\u0641\u0636\u0644 \u062f\u0643\u062a\u0648\u0631 \u0627\u0648\u0631\u0627\u0645<\/bdi> (best oncologist) dropped out of the top 50 in September 2025.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>12 of the 107 priority keywords were outside the top 50 in October 2025. Six never entered it, including breast cancer and ADHD terms, and six reached it and fell back out.<\/li>\n\n\n<li>From May to August 2025, several #1 keywords, among them <bdi lang=\"ar\">\u0627\u0641\u0636\u0644 \u0645\u0633\u062a\u0634\u0641\u0649 \u0641\u064a \u0627\u0644\u062f\u0645\u0627\u0645<\/bdi> and <bdi lang=\"ar\">\u0645\u0633\u062a\u0634\u0641\u0649 \u0641\u064a \u0627\u0644\u0645\u062f\u064a\u0646\u0629 \u0627\u0644\u0645\u0646\u0648\u0631\u0629<\/bdi>, slipped to between 3 and 5 before returning to the top 2 by October. Our records do not show a single clear cause.<\/li>\n\n\n<li>We specified an llms.txt file in September 2025. As of October 2026, mouwasat.com\/llms.txt returns an empty file, so none of the AI results above depend on it.<\/li>\n\n\n<li>Clicks from Google grew 1.3% year on year while impressions grew 33.2%. Visibility is up sharply, but more of it is being answered on Google&#8217;s own page.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">These gaps are where the next round of work goes: refreshing the internal medicine and oncology pages, and building content for the six terms that have never ranked.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What can other hospitals copy from this?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Most of what worked here transfers to any hospital or clinic group in the Gulf. The order matters as much as the tactics: measure first, fix the technical base, map one intent to one page, then build content and authority, and track AI visibility as its own channel.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Track a fixed list of high-intent keywords every month.<\/strong> Brand traffic hides everything. A 107-keyword list made the non-brand gap impossible to ignore.<\/li>\n\n\n<li><strong>Give every city and specialty its own page.<\/strong> City plus intent searches, such as hospitals in Dammam or best hospital in Madinah, carried the biggest commercial wins.<\/li>\n\n\n<li><strong>Clean the link profile early.<\/strong> The disavow file went in during month 4, on 26 March 2024.<\/li>\n\n\n<li><strong>Write patient guides as complete answers in both languages.<\/strong> They are what Google&#8217;s AI features cite most.<\/li>\n\n\n<li><strong>Report AI visibility separately.<\/strong> Search Console&#8217;s generative AI report and an AI citation tracker show a channel that classic rank tracking misses.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">For the summary version of this project, see the <a href=\"\/case-studies\/al-mouwasat\/\">Al Mouwasat case study<\/a>.
